Referee Payment:
Before we can issue you a check for payment, we need a completed 2008 W-9 form. We will NOT be withholding taxes but we need this form for other filing purposes. Follow this link for a form and instructions - http://www.irs.gov/pub/irs-pdf/fw9.pdf
Background Checks:
We are also required to have background checks completed for everyone who coaches, referees, or volunteers. Please fill out the form at this link:
